How to Register for Michigan FAR Flyers Varsity Hockey Team:

Below is the page on the FAR Conservatory web site where you can retrieve the forms you need to sign up for the team. I've also placed direct links to each of the required forms below. You can download or print them.

Please make sure to complete all sections and turn them into the office prior to the start of our season on Saturday, September 10, 2011. You can turn them in to me (Head Coach Ben Niemiec) at the rink, too. All players will have to be officially signed up before they are allowed on the ice. Officially means the office has notified me that they have the player's required paperwork or you have given it to me before the practice.

Each player needs to turn in the first three forms below (Class Registration, Yearly Enrollment and Code of Conduct). Make sure to fill out both pages on the Class Registration form.
